Immunizations are available by appointment. 

Please call (636) 528-6117 to find out if you or your child are eligible to get immunizations at our office and schedule an appointment.

 

The Lincoln County Health Department (LCHD) Immunization Clinic offers all childhood and adult vaccines that are recommended by the CDC and the State of Missouri. 

We offer immunizations for people that:

  • Do not have health insurance

  • Are on state Medicaid

  • Have other health insurance

To be prepared for your appointment:

  • All patients under 18 should be accompanied by an adult or guardian, or have an adult or guardian complete the following forms before the appointment: Click Here for Online Forms

  • You must bring a current immunization record to the visit

  • You must bring insurance information or an insurance card to each visit (if you have insurance)

  • Please arrive 15 minutes early to fill out the required forms, or complete forms online: Click Here for Online Forms

The state of Missouri has state statutes requiring an up-to-date immunization record for all children entering public and private daycare centers, preschools and grade school.

VFC and 317 Vaccines are provided by the Missouri Department of Health and Senior Services (DHSS) and are purchased in part with federal funds through the CDC and the Vaccines for Children (VFC) program.
